Once you have filled out this form, the CQA/ACC Bookkeeper will be sending an invoice to the email address you have filled out below.

Invoices will be sent between late September and mid-October. Payment is due by October 31, 2026, for insurance coverage to start on November 1, 2026.

Payment options—credit card, PayPal, e-transfer, and cheques—will be included with the invoice. Look for the full payment details in the invoice email.

Premium cost for this program will be added to this page soon. invoices won’t be processed until after we get pricing.
