Quilt Canada Guild Sponsorship Form

Making Your Sponsorship Payment:

After you hit the submit button, you can make your payment in one of three ways:

1. Print PDF form and send with cheque made out to CQA/ACC (address to send to is on the PDF form)

2. E transfer amount of donation to brenda.horvath@canadianquilteradministration.com with message "Quilt Canada Guild donation"

3. Click on the link to Guild Sponsorship in the SHOP area of the website and choose your level and pay via the website by credit card.

Once you hit submit, you will receive a email acknowledging your sponsorship form submission. On the email will be instructions for making the payment to CQA/ACC.
